In patients with Unruptured Intracranial Aneurysm. Does treatment of the aneurysm reduce the frequency and severity of headaches?

Resumo

Introdução: A cefaleia é o sintoma que caracteriza um aneurisma intracraniano não roto em aproximadamente um terço dos casos. Após o tratamento, podem ocorrer alterações no padrão da cefaleia. Este estudo tem como objetivo analisar se, em pacientes com aneurisma intracraniano não roto, o tratamento do aneurisma reduz a frequência e a intensidade das cefaleias. Métodos: Revisão sistemática usando o PubMed como base de dados para identificar a literatura relevante entre 2002 e 2022. Foram selecionados artigos escritos em inglês, espanhol e português, limitados a estudos clínicos, de coorte, transversais e de caso-controle. Os dados obtidos foram analisados e ponderados, com 115 artigos incluídos e 84 excluídos. Resultados: 70% dos artigos estudados relatam uma redução na frequência e intensidade da cefaleia após o reparo cirúrgico de aneurismas não rotos. Dos estudos selecionados, nenhum indica o tratamento em relação à cefaleia. 30% dos artigos não indicam a concordância entre dor e tratamento. Conclusão: O tratamento endovascular de aneurismas não rotos é factível e favorável à diminuição da intensidade da cefaleia. No entanto, essa não é uma decisão unânime entre os pesquisadores, assim como o tempo necessário após o tratamento cirúrgico para que uma melhora significativa seja observada.

Abstract

 Introduction: Headache is the symptom that characterizes an unruptured intracranial aneurysm in approximately one-third of cases. After treatment, changes in the headache pattern may occur. This study aims to analyze whether, in patients with unruptured intracranial aneurysm, treatment of the aneurysm reduces the frequency and intensity of headaches. Methods: Systematic review using PubMed as a database to identify relevant literature between 2002 and 2022. Articles written in English, Spanish and Portuguese, limited to clinical, cohort, cross-sectional and case-control studies were selected. The data obtained were analyzed and weighted, with 115 articles included and 84 excluded. Results: 70% of the articles studied report a reduction in the frequency and intensity of headache after surgical repair of unruptured aneurysms. Of the selected studies, none account indicates the treatment in regards to the headache. 30% of the articles do not state the agreement between pain and treatment. Conclusion: Endovascular treatment of unruptured aneurysms is feasible and favorable for decreasing the intensity of headaches. However, this is not a unanimous decision among researchers; as well as the time required after surgical treatment for significant improvement to be observed.
