Intracranial Chordomas and Chondrosarcomas: preliminary study

Resumo

Introdução: Cordomas intracranianos e condrossarcomas são lesões desafiadoras na cirurgia da base do crânio, e diferentes abordagens cirúrgicas podem e devem ser usadas para o manejo desses tumores. Objetivo: Discutir os resultados cirúrgicos de pacientes com cordomas e condrossarcomas de base do crânio, tratados pelos autores, detalhando nossa abordagem de manejo para cada caso e revisando os desafios distintos trazidos por estes tumores complexos. Métodos: Catorze pacientes com cordomas e condrossarcomas foram operados pelos autores entre 2007 e 2023. Resultados: A ressecção total e a ausência de novos déficits foram alcançadas em quase todos os pacientes. Conclusão: Estes tumores podem ser tratados com sucesso com o conhecimento das abordagens da base do crânio, o uso de um neuronavegador em casos selecionados, e o aspirador ultrassônico, particularmente em cordomas.

Abstract

Introduction: Intracranial Chordomas and chondrosarcomas are challenging lesions in skull base surgery, and different surgical approaches can and should be used for the management of these tumors. Objective: To discuss surgical outcomes of 14 patients with skull base chordomas and chondrosarcomas treated by the authors detailing our management approach for each case and reviewing the distinctive challenges posed by these complex tumors. Methods: Fourteen patients with chordomas and chondrosarcomas were operated on by the authors between 2007 and 2023. Results: Total resection and the absence of new deficits were achieved in almost all patients. Conclusion: These tumors can be successfully treated with the knowledge of skull base approaches, the use of a neuronavigator in selected cases, and the ultrasonic aspirator, particularly in chordomas.
