Complications of Ventriculoperitoneal Shunt: a systematic review on subdural hematomas

Resumo

Introdução: A cirurgia de derivação ventrículo-peritoneal (DVP) é um tratamento comum para hidrocefalia, redirecionando o líquido cefalorraquidiano (LCR), mas pode levar a complicações como hematomas subdurais, afetando negativamente o prognóstico do paciente. Este relato inclui um caso ilustrativo de um paciente do sexo masculino, 68 anos, que desenvolveu hematomas subdurais bilaterais após VPS. A intervenção cirúrgica foi bem sucedida. Métodos: Foi realizada uma revisão sistemática no PubMed (2014-2024), com foco na incidência, fatores de risco e estratégias de manejo clínico. Resultados: A análise destacou fatores-chave como técnica cirúrgica, presença de coagulopatias e diagnóstico precoce por TC/RM como críticos para a obtenção de desfechos favoráveis. Discussão: Fatores relacionados ao paciente, incluindo idade avançada e uso de anticoagulantes, aumentam significativamente o risco. Estratégias preventivas e intervenções oportunas podem melhorar os resultados clínicos. Conclusão: Os hematomas subdurais são complicações graves após a cirurgia de DVP. O diagnóstico precoce e o manejo clínico adequado são essenciais.

Abstract

Introduction: Ventriculoperitoneal Shunt (VPS) surgery is a common treatment for hydrocephalus, redirecting cerebrospinal fluid (CSF), but it can lead to complications such as subdural hematomas, adversely impacting patient prognosis. This report includes an illustrative case of a 68-year-old male patient who developed bilateral subdural hematomas following VPS. Surgical intervention was successful. Methods: A systematic review was conducted on PubMed (2014-2024), focusing on the incidence, risk factors, and clinical management strategies. Results: Analysis highlighted key factors such as surgical technique, presence of coagulopathies, and early diagnosis through CT/MRI as critical for achieving favorable outcomes. Discussion: Patient-related factors, including advanced age and anticoagulant use, significantly increase risk. Preventive strategies and timely interventions can enhance clinical results. Conclusion: Subdural hematomas are serious complications following VPS surgery. Early diagnosis and appropriate clinical management are essential.
