Resumo

O cisto ósseo aneurismático (CBA) é uma lesão benigna, porém localmente agressiva, que afeta principalmente a população pediátrica e adulta jovem. No contexto dos tumores ósseos, a avaliação inicial do ABC requer radiografias, seguidas de ressonância magnética (RM) ou tomografia computadorizada (TC) para caracterização posterior. Este relato de caso elucida uma apresentação atípica do cisto ósseo aneurismático, lançando luz sobre diagnósticos diferenciais e nuances clínicas que evocam a suspeita de um curso não convencional da doença. Sexo masculino, 30 anos, previamente hígido, queixa principal foi obstrução nasal progressiva há 6 meses, epistaxe esporádica. Notavelmente, houve ausência de dor associada a esses sintomas. No caso específico mencionado, o tratamento nasal endoscópico foi empregado para tratar o osso esfenoidal fraturado. O deslocamento orbital, o desvio de septo e a erosão observados de estruturas adjacentes reforçam a necessidade de uma abordagem multidisciplinar no manejo desses casos.

Abstract

The Aneurysmal Bone Cyst (ABC) is a benign yet locally aggressive lesion primarily affecting the pediatric and young adult population. In the context of bone tumors, the initial assessment of ABCs necessitates radiographs, followed by magnetic resonance imaging (MRI) or computed tomography (CT) for further characterization. This case report elucidates an atypical presentation of Aneurysmal Bone Cyst, shedding light on differential diagnoses and clinical nuances that evoke suspicion of an unconventional disease course. A male, 30 years-old, previously healthy, presented as his chief complaint a progressive nasal obstruction for 6 months, with sporadic epistaxis. Notably, there was an absence of pain associated with these symptoms. In the specific case mentioned, endoscopic nasal treatment was employed to address the fractured sphenoid bone. The observed orbital displacement, septal deviation, and erosion of adjacent structures underscore the need for a multidisciplinary approach in the management of such cases.
